I know that the patient discount plan is ignored if the patient has insurance attached to their record. If the patient doesn't have insurance attached, I'm not sure what could be going on.

Did you associate the provider with aetna?  Under User Roles, Fees, you say they are contracted with Aetna.

If the patient has an insurance plan attached, there is no need to include the discount fee schedule under the patient information since that field will not be used. The provider also  needs to be marked as "Contracted with" in the provider user account. The insurance fees for the carrier are not posted on the ledger, only the office fees are posted on the ledger. In network write-offs are posted when a payment is received and posted.

If the patient does not have any insurance attached, the discount fee schedule can be placed in the discount field under the patient information. When a procedure is posted, the office fee will post for the procedure and then a discount adjustment will be posted reflecting the difference between the office fee and the discount fee.

In the event that the patient has an insurance plan and then a discount plan in addition to the insurance plan, only the insurance plan fee schedule will be used. Currently there is not a way to take into account the additional discount plan fees.
