You can customize the consent message that appears on treatment consent forms. A patient's consent form will have one of the financial disclaimers, the default consent message, and any custom messages. Each location in your organization can have a different consent message.
To customize treatment consent messages
How to get there
If you are not already viewing the correct location, select it on the Location menu.
On the Settings menu, under Location, click (or tap) Patient Forms.
The Patient Forms page opens.
On a location's Patient Forms page, under Tx Consent Messages, select Consent for Dental Treatment.
The options for editing the message become available.
Customize the text that will appear.
To not include a consent message, delete any text in the box.
To return the box's text to the system default, click (or tap) the Reset to default link.
There is no limit to the length of the message.
Click (or tap) Save.
