Patient forms provide a streamlined process for patients to update their medical history or provide you with their insurance information. The link to fill out forms electronically can be texted or emailed to a patient. If the patient arrives for an appointment and his or her forms have not been filled out, they can be manually sent to the patient via email or text right then and there.

- Once a form is filled out, it immediately appears in the Patient Forms section of the patient Overview page.
- If a form is not due, it cannot be requested for a patient. You would have to change the expiration setting in order to require that form be filled out again.
