Adding an Addendum to a Signed Clinical Note

Occasionally you’ll find mistakes in your clinical notes or that your notes are incomplete; therefore, you’ll need to correct the mistakes. However, if the clinical note has been signed, you cannot change the note. In this situation, you must add an addendum to the note and make the changes in a new note, which is appended to the original note.
