You can customize the financial disclaimer (Insured/Uninsured Disclaimer) that appears at the bottom of printed treatment plans and on treatment consent forms. The disclaimer for insured patients is separate from the one for uninsured patients. Each location in your organization can have a different set of financial disclaimers.
Note: A printed treatment plan or a consent form will have one of the financial disclaimers (Insured/Uninsured Disclaimers) depending on whether the patient is insured or not, the Consent for Dental Treatment message, and any applicable custom treatment consent messages.
To customize financial disclaimers
How to get there
If you are not already viewing the correct location, select it on the Location menu.
On the Settings menu, under Location, click (or tap) Patient Forms.
The Patient Forms page opens.
On the a location's Patient Forms page, under Tx Consent Messages, select Insured/Uninsured Disclaimers.
The options for editing the disclaimers become available.
Set up the following options:
For patients with insurance - Customize the text that will appear if a patient has insurance.
For uninsured patients only - Customize the text that will appear if a patient does not have insurance.
To not include a disclaimer for insured patients and/or uninsured patients, delete any text in the applicable box.
To return any box's text to the system default, click (or tap) the corresponding Reset to default link.
There is no limit to the length of the disclaimer, but (depending on the number of treatment-planned procedures, the length of any included notes, and the length of the disclaimer) all of the disclaimer's text might not fit on the last page of the printed treatment plan.
Click (or tap) Save.
